On 5/30/25 at 12:30 PM, Licensing Program Analyst (LPA) Keturah Lane conducted an unannounced case management visit to follow up on an unusual incident reported by the facility on 4/28/25. The incident involved a child (C1) who sustained an injury on the playground. Upon arrival, LPA met with staff member Margaret Noll and lead teacher Gabriela Padilla and toured the facility. LPA observed a total of 52 children with the following ratios during naptime: Whales classroom (Toddlers) had 10 toddlers with staff member Shannon Gallagher Walrus classroom (Toddlers) had 8 toddlers with staff member Justyna Feldman Shark classroom had 13 children with staff member Charlyn Slaughter Octopus classroom had 9 children with staff member Isabelle Quejada Dolphins classroom had 12 children with staff member Catherine Leos LPA inspected the playground and observed a large canopy with four large wide poles. One of the poles has a foam covering around the pole, which was installed shortly after the incident. The facility is in the process of covering the other 3 poles and should be completed in the next couple weeks. LPA did not observe any sharp or protruding parts from any of the poles. Other poles in the playground have been covered that have a smaller radius. It has been a challenge to find a wide enough covering to go around the poles. Staff stated that they have never had an incident regarding the canopy poles before. It appears to have been an unfortunate accident. Facility stated there were 15 children on the playground at the time of the incident with 3 staff members supervising. Director Nora Camacho arrived at the facility at approximately 1:25 PM. No deficiencies cited at this inspection. Exit interview conducted and report was reviewed with Director Nora Camacho. Notice of site visit was provided and must be posted for 30 days.
