What the inspector wrote

On September 22, 2025, at 8:50 a.m., Licensing Program Analyst (LPA), Angela Nguyen and Licensing Program Manager (LPM) Rajani Goudreau conducted an unannounced Annual inspection. LPA met with Facility Representative, LeAnne Talada and disclosed the purpose of inspection. LPA were led on a tour of the inside and outside of the facility by the Facility Representative. There were six (6) infants being supervised by two (2) staff members in the infant room. The facility serves age birth \342\200\223 24 months in the infant room. Hours of operation are Monday \342\200\223 Friday 6:45 a.m. to 5:15 p.m. Disinfectants, cleaning solutions, and other hazardous items were made inaccessible to the children in care. Furniture and equipment in the classroom was observed to be in good condition, free of sharp, loose or pointed parts. Infant changing table were within arms reach of the sink inside the facility. LPA observed a changing mat, diapers and opened package of wipes on top of a storage bin outside in front of the outdoor play area. Potty chairs, toilet and hand washing equipment are in safe and sanitary operating condition. The surface of the outdoor activity space and playground equipment is maintained, is in safe condition and is free of hazards. There is sufficient cushioning to absorb falls under and around play equipment. The playground area includes a shaded rest area for children through the use of canopies shades. Areas around high climbing equipment were observed to have sufficient cushioning material by the use of artificial grass to absorb falls. Drinking water is available both indoors and outdoors. Facility Representative stated that there are no swimming pools or other bodies of water on the premises. LPA did not observe any bodies of water. Facility Representative stated there are no firearms or ammunition allowed or stored on the premises. The facility has a functioning carbon monoxide detector and a first aid kit that meet statutory requirements. The last fire drill was conducted and documented on 1/17/2025. Earthquake drill was conducted on 1/17/2025. LPA advised Facility Representative that fire and disaster drills shall be conducted every 6 months. Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Capacity and limitations as specified on the license are being maintained. Children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than four (4) children in care. The name of the facility representative and the fully qualified teacher designated to act in the facility representative\342\200\231s absence has been reported to the Department. LPA reviewed facility physical sign in /sign out sheets. 2 out 6 children were not signed into the facility. Facility Representative was reminded that the person who signs the child in and out of the facility shall use their full legal signature and record the time of day. LPA reviewed a sample of children\342\200\231s files and observed the files were complete with contact information for authorized representative and/or relatives or others who can assume responsibility for medical assessment. 2 out of 6 children's files were incomplete, missing medical assessment and immunization records. Bottles containing formula contain the child\342\200\231s name and date. Infant Needs and Service Plans for all children in care were not updated quarterly. LPA observed 15-minute sleep checks documented within 15-minute intervals. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility. LPA reviewed a sample of staff files and observed files were complete with health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility Representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. LPA reviewed with Facility Representative, the LIC 311A, Records to Be Maintained At The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. LPA and Facility Representative discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, Mandated Reporter Training, Forms and Regulations, Safe Sleep in Childcare, Lead Poisoning Facts, Guardian and California Megan\342\200\231s Law ( www.meganslaw.ca.gov ). Four (4) Type B Deficiencies per California Code of Regulations, (Title 22, Division 12 & Chapter 1), are being cited on the attached LIC 809-D. A Technical Violation (TV) and Technical Advisory (TA) was issued during the inspection. A Notice of Site Visit (LIC 9213) was given to Facility Representative, LeAnne Talada and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100 An exit interview was conducted and the report was reviewed with Facility Representative, LeAnne Talada.
