What the inspector wrote

On October 16, 2025 at 11:11 a.m., Licensing Program Analyst (LPA), Angela Nguyen conducted an unannounced Plan of Correction (POC) inspection at the facility. Purpose of this inspection is to ensure deficiencies cited during a Annual inspection dated September 22, 2025 were corrected. Upon arrival, LPA met with Facility Director, LeAnne Talada and proceeded to tour the facility. There were 7 infants being supervised by 2 staff members, during today\342\200\231s inspection. The following deficiencies were corrected as followed: \302\267 Facility Director ensured children files were complete with immunization records and physician reports for Child #1, Child #2. LPA reviewed files and determined to be completed. \302\267 Facility Director provided the Needs and Services plans to the child's authorized representative. LPA reviewed child's files and determined to be completed. \302\267 LPA reviewed sign in/out procedure and determined it to be complete. No deficiencies cited. LPA provided Facility Director with Cleared POC letters. A Notice of Site Visit (LIC 9213) was given to Facility Director, LeAnne Talada and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted and the report was reviewed with Facility Director, LeAnne Talada.
