What the inspector wrote

On 5/7/2025 at 12:31pm, Licensing Program Analyst (LPA) Vicky Williamson conducted an unannounced case management inspection to cite for deficiencies observed during another inspection at the facility. LPA met with Facility Representative, Celia Carrizosa. There were 16 children present with five (5) staff members. LPA's review of staff files determined that AB1207 Mandated Reporter certifications for Staff #1 (S1) and Staff #2 (S2) were not current. Per California Code of Regulations, Title 22, Division 12, Chapter 1, one (1) Type B deficiency is being cited on the attached LIC 809D. A Notice of Site Visit (LIC 9213) was given to Facility Representative, Celia Carrizosa and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Appeal Rights (LIC 9058) was provided. Exit interview conducted and report was reviewed with Facility Representative, Celia Carrizosa.
