What the inspector wrote

On November 3, 2023 at 8:35 a.m. Licensing Program Analyst (LPA) Leilani Curtis conducted an unannounced case management inspection for the purpose of delivering an amended report, LIC 9099 and LIC 9099D from an original report dated 10/16/2023. Upon arrival LPA met with Assistant Director Ana Pacheco and proceeded to tour the facility. During this inspection there were 41 children present with 8 staff members. Appropriate teacher and child ratios were observed. LPA observed staff members Ashley Beauregard and Genet Boru working with children without being associated to the facility. LPA contacted the regional office and based on a review of the Guardian Background Check System, the staff members are clear and eligible to work but are not associated to the facility. The director Monica Collins arrived at approximately 9:15 a.m. The director states that Ms. Beauregard began working at the facility on 5/16/23 and Ms. Boru began working at the facility on 4/4/23. LPA obtained the appropriate documents to clear the deficiency cited for out of ratio on LPA Curtis's complaint investigation report dated 10/16/23. The deficiency cited on the complaint investigation report for commingling the infant/toddler children with the preschool children has not been cleared. See LIC809D for cited deficiency. A civil penalty has been assessed. An exit interview was conducted with Director Collins and appeal rights (LIC 9058) were discussed. A copy of this report as well as a copy of the appeal rights were given to the Director. The director’s signature on this form acknowledges receipt of these rights. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS. LPA observed the director post notice of site visit.
