What the inspector wrote

On 12/12/24 at 11:45 am, Licensing Program Analyst (LPA) Gerald Poindexter conducted an unannounced case management inspection while at the facility on another matter. LPA met with Director Monica Collins and discussed the regulation related to that other matter and the related incident – a classroom window was recently broken from contact with a child ­– which presents a potential health, safety, and personal rights risk to children in care. That incident was not reported to the Department by the facility. Additionally, based on staff interview, the LPA was informed of an earlier broken window incident from September 2024 (child threw water bottle) that was not reported. Neither incident resulted in injury to children in care. See LIC 809D for deficiency cited. Exit interview conducted and report was reviewed with the director Monica Collins. A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal rights were provided..

This requirement was not met as evidenced by...On 12/5/24 a classroom window was contacted by a child and observed to be cracked by facility staff. In September 2024, a window was cracked by a child throwing an object. The facility did not provide an Unusual Incident Report for either incident to the Department according to requirements above, which is a potential health, safety and personal rights risk to children in care.
