On 11/14/25 at 12:56 pm, LPA Sharon Mendez and LPA Nancy Diaz made an unannounced visit for the purpose of annual inspection. LPAs met with Acting Director Daisy Bautista, and toured the facility. There were 20 children present with 5 teachers in 2 rooms. Facility is within ratio and capacity. LPA toured the facility. The rooms were clean and in a comfortable temperature during the visit. Adequate ventilation and heating are available. All required forms were posted. The furniture, books, games, and toys are safe, and age appropriate. There is a variety of activities provided throughout the day. The infant playground is fully fenced and separate from other programs with age-appropriate play equipment and adequate shade. On the playground there is no sufficient cushioning and has holes wich causes tripping hazard for the children in care, LPA advise acting director that this needs to be corrected see 809D for deficiency sited. Drinking water is available at all times. Children are evaluated upon entry and monitored throughout the day for signs of illness. The isolation area for ill children awaiting pick up is the director’s office. The children’s food is stored in the kitchen, which is clean and orderly. There is no expired or contaminated food present. The food has been stored separately from any chemicals or cleaning products. C ontinued on page 2... The menu is posted but not updated acting director was reminded to keep it updated and to record any changes and to stored for 30 days. LPAs observed 3 infants sleeping not swaddled and with nothing hanging from the cribs according to regulation. Infant changing tables have a padded surface not less than one inch thick and are sanitized regularly, blankets are sent home with the parent to wash thoroughly. Infant bottles are label per child. Hand washing and toileting areas are in a safe, sanitary, and operating condition. There are staff present with current CPR and First Aid certification. Medications are stored in the director’s office under key and is inaccessible to children with all required documentation on file. There is no evidence of rodent or insect activity. The carbon monoxide detector and fire alarm are operational. The facility has a written disaster plan in place, and they have been conducting and documenting evacuation drills every month months. The facility does not transport children. LPA reviewed sign in/out sheets, a sample of personnel records and a sample of children's records. The acting director was reminded that Mandated Reporter Training is to be retaken every two years and can be accessed at the following website: www.mandatedreporterca.com . LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed the Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Continued on page 3... A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance with the Written Directives outlined in PIN 21-21.1-CCP. Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.Licensee is advised to sign up for Quarterly Updates and Provider Information Notices (PINs) for one or more programs on our website: www.ccld.ca.gov . Select “Child Care” then “Quick Links” and Quarterly Updates.
