Correction check·December 2, 2025

On 12/03/2025 at 9:50AM Licensing Program Analyst (LPA) Sharon Mendez and (LPA) Evelyn Reyes conducted an unannounced Plan of Correction visit for the purpose of clearing a deficiency previously cited on 11/14/2025 and to provide an amended report for a report previously provided on the same date 11/14/2025. Upon arrival, LPA met with Director Ana Pacheco and toured the facility and playground to make sure the cushioning material was repaired. LPA observed the holes in the cushioning playground patched up and repaired to an even surface eliminating the tripping hazard for the children in care. There are 56 children and 9 staff. Facility is within Ratio. No deficiencies were cited at this visit. Exit interview conducted and report was reviewed with Director Ana Pacheco . Notice of site visit was provided and must remain posted for 30 days.
