What the inspector wrote

Licensing Program Analysts (LPAs) James Wilkerson & Jeanette Sanchez arrived at this facility to follow-up on an incident report that occurred on 01/23/23. An Unusual Incident Report (UIR) indicates that a child ran from a playground through a latch gate and back inside the facility, ran down the long hallway, out the front door into the parking lot. The child made it about halfway into the parking lot before turning around and heading back towards the facility. The parking lot is next to College Blvd., which is a very busy thoroughfare. Staff indicate that the entire incident was observed by staff who chased after the child the entire way and that staff never lost sight of the child. Staff caught the child as the child circled back toward the facility and was brought back inside the facility. During this visit today, staff showed LPA Wilkerson the back playground which children can enter from classroom #5. The children were being transitioned from the back playground to the front playground which is closest to the street. Information received during this visit is that a staff member was standing by the gate and the door leading to classroom #5 which are next to each other. The gate has a latch on it to make it inaccessible. The gate leading from the back playground has a latch as well. The child ran to the back of the building and opened the door leading to the long hallway. The child took off running down the hallway while be pursued by a staff member. When the child got close to the front door leading to the parking lot another staff member caught sight of the incident. SEE 809D for deficiency cited. SEE NEXT PAGE. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility for the next 12 months. The Notice of Site Visit and Type A Deficiencies from today’s visit must be posted for 30 days. Failure to keep these posted for the entire 30 days will result in an immediate $100 civil penalty for each. An exit interview was conducted, appeal rights discussed and will be provided along with from LIC 9224 (AB633), a Notice of Site Visit and a copy of this report .

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    Responsibility for Providing Care and Supervision - (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time. This requirement was not met as evidenced by: A child ran from a playground, through a gate, through the back door of the facility, down the hall inside the parking lot while being pursued by staff. The child stopped running and turned around toward the front door of the facility before being brought back inside. This poses an immediate risk to the health and safety of the children.
