What the inspector wrote

On 06/03/2026, Licensing Program Analyst (LPA), Tricia Danielson conducted an unannounced case management visit to follow up on Unusual Incident Report (UIR) that the facility self reported to the Department on 05/22/2026 concerning an incident that took place on 05/21/2026. LPA met with Director Wilma Cabrera to discuss the incident. At the time of today's visit, the facility had 62 preschool children enrolled and 54 were present. Director Cabrera reported that at approximately 10:00 AM on 05/21/2026 during transition from the playground to the Pre-K classroom, Staff #1 (S1) was observed by a parent to be speaking to Child #1(C1) in an angry manner. S1 was also observed to have physically forced C1 down to the floor by placing both hands on the shoulders of C1 and forcefully encouraging C1 down toward the floor causing C1 to land in a squatting position against a wall. It was also observed that once C1 was in the squatting position, S1 pulled C1's legs out from under them which caused C1 to land on their bottom. S1 was then observed to physically force C1 into a cross legged position. Cabrera reported the parent confronted S1 and informed staff of their observations. C1 was evaluated for injuries by staff and was found to be injury free. C1's parents were notified of the incident as well. S1 was removed from the center following the incident and placed on administrative leave. There are no citations noted at this time as the center acted appropriately in the handling of the incident based on the information provided during today's visit. An exit interview was conducted, and this report was reviewed with and provided to Director Cabrera. Appeal Rights were also discussed and provided. An LIC 9213- Notice of Site Visit was also issued and must remain posted near the main entrance for 30 days. Non-compliance with posting will result in a $100 fine. This report must be accessible to the public for three years.
