What the inspector wrote

Licensing Program Analysts (LPAs) James Wilkerson & Jeanette Sanchez arrived at this facility to follow-up on an Unusual Incident Report (UIR) that occurred on 02/06/23. On that date children were being transitioned from the Toddler Classroom to the Infant "A" Classroom. A child was left unattended for what the facility stated was less than one minute. The child was left in the hallway alone and was found by a parent who notified staff. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility for the next 12 months. The Notice of Site Visit and Type A Deficiencies from today\342\200\231s visit must be posted for 30 days. Failure to keep these posted for the entire 30 days will result in an immediate $100 civil penalty for each. SEE LIC 809D for deficiency cited. Additionally, this is the second citation for the same regulation within a 12 month period (09/29/22) and will result in a $250.00 Civil Penalty. SEE LIC 421*FC A Civil Penalty has been assessed on this visit. Payment is due when billed and the check(s) or money orders shall be made payable to the \342\200\234California Department of Social Services\342\200\235. YOU WILL RECEIVE AN INVOICE IN THE MAIL. DO NOT SEND MONEY UNTIL YOU RECEIVE YOUR INVOICE. DO NOT SEND CASH. An exit interview was conducted, appeal rights discussed and will be provided along with form LIC 9224 (AB633), a Notice of Site Visit and a copy of this report.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    Responsibility for Providing Care and Supervision - (a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by: A child was left alone in the hallway during transition from the Toddler Classroom to the Infant "A" Classroom and was found by a parent who notified staff. This poses an immediate risk to the health and safety of the children.
