Photo evidence of knife obtained. The evidence is unclear if the knife was accessible to children in care. Typically knives are stored in an inaccessible area. The restroom the children in care use, requires for children to pass through the safety gate located by the kitchen. Based on observations and interviews children are supervised when passing through the gate to use the restroom. Investigative Interviews conducted, contradictory information was provided regarding hazardous items are accessible to children in care. Although the allegation may have happened or is valid, there is a not a preponderance of evidence to prove that the alleged violation occurred. Therefore, the above allegation is found to be unsubstantiated. Exit interview conducted and report was reviewed with the Director Rosa Prieto. A notice of site visit was given and must remain posted for 30 days.
