What the inspector wrote

On 08/25/2025 at 8:20AM, Licensing Program Analysts (LPAs) Adriana Macias and Sharon Mendez conducted an unannounced case management inspection to follow up on a self-reported unusual incident. The incident was reported to licensing on 7/31/2025, and it involved a child (C1) who stated that a staff member (S1) had violated C1's personal rights. Upon arrival, LPAs met with Director Rosa Prieto, explained the reason for visit and toured the facility. A total census of 17 children and 3 staff with appropriate ratios were observed. All staff have fingerprint clearances. During inspection, LPA\342\200\231s interviewed director, staff and children. Director provided LIC 500 Personnel Report, and Children\342\200\231s Roster. Per director and C1\342\200\231s mom, it is unknown exactly when the incident happened, but C1 recently disclosed the incident on 7/16/2025, but there are no witnesses or similar incidents reported by other parents or children. S1 was let go, prior to C1\342\200\231s disclosure, for other reasons unrelated. No deficiencies were observed during inspection. Exit interview conducted and report was reviewed with Director Rosa Prieto. LPAs provided a copy of the report and appeal rights. A notice of site visit was provided and must remain posted for 30 days.
