What Happened
Complaint investigation substantiated multiple failures in reporting and child care. Staff admitted to no drinking water for children for 1-3 days in early June - only milk provided at meals while Site Supervisor was aware. Potential gas leak occurred months prior with facility remaining closed for several hours; kitchen stove requires replacement. No Unusual Incident Reports filed for either incident and parents not notified.
Observations
- No drinking water available for children for approximately 1-3 days in early June; staff provided only milk at breakfast and lunch while Site Supervisor was aware
- Potential gas leak occurred months prior; SDG&E cleared facility to reopen; kitchen stove needs replacement
- Part-day teacher notified afternoon program parents not to attend based on incident but other parents were not informed
- Director not on site at time of incident and no longer available for interview
- No Unusual Incident Reports received for lack of drinking water incident
- No Unusual Incident Reports received for potential gas leak incident
- Parents were not notified of either incident - substantiated

Reporting Requirements:(d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (1) Events reported shall include the following: (C)Any unusual incident or child absence that threatens the physical or emotional health or safety of any child.
Drinking Water:(a)Drinking water from a noncontaminating fixture or container shall be readily available both indoors and in the outdoor activity area.(1) Children shall be free to drink as they wish.
Buildings and Grounds:(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.
