What Happened
Complaint investigation substantiated. Child #1 exhibited behaviors causing harm to staff and children since enrollment in 12/2024, documented in multiple case notes with photographic evidence. Despite behavioral plan and referral provided to parents, child continued exhibiting same behaviors with no additional supports implemented by facility.

This requirement was not met as evidenced by. Based on interviews conducted and record review, Child #1 (C1) has exhibited behaviors in the classroom that caused harm to staff and children, even though case notes were completed and a Behavioral Plan was created, C1 still exhibits same behavior and no additional supports have been in place. This is a potential health and safety risk to children in care.
