On 10/4/22 at 9:15AM, an unannounced Case Management Incident Inspection was made to follow up on a self-reported incident that occurred on 9/22/22 by Licensing Program Analysts (LPAs) Saraliz Velando and Adrian Mangina. Upon arrival, LPAs met with Assistant Director, Tina Thomas. At approximately 10:15AM, Director, Sandy Cook arrived to the facility and met with LPAs. The following ratios were observed in the facility, there were 112 children to 13 staff members. In the Willems Room, 12 children were being supervised by Annie VanWyk in the playground and 10 children were being supervised by Preeti Sinha in the classroom. The Silverstein Room had 9 children being supervised by Rose Palmer outside and 12 children supervised by Wendy Corona inside. The Dean Room had 20 children and was being supervised by Angel Au and Ausra Bunas. The Potter Room had 14 children and was being supervised by Minjung Song and Erin Kohn. The Seuss Room had 11 children being supervised by Christine Smith and Alison McCann. The Milne Room had 12 children and was being supervised by Adriana Terzoli and Whitney Gonzalez-Shaver. The Carle Room had 12 children and was being supervised by Cameryn Talpas and Melody De Galicia. Staff members have the required background clearances and are associated to the facility. The facility self-reported the incident to the department by telephone on 9/26/22 and it was reported in a timely manner. The facility reported that a parent informed them that their child may have been injured while in care. LPAs interviewed staff and further information is needed to determine if a violation occurred. If it is later determined that a violation occurred, a subsequent visit will be made to issue a citation. Exit interview conducted and report was reviewed with the director, Sandy Cook. A notice of site visit was given and must remain posted for 30 days.
