On 11/9/22 at approximately 2:50pm, an unannounced Case Management Incident Inspection was made to follow up on a self-reported incident that occurred on 10/27/22 by Licensing Program Analysts (LPAs) Saraliz Velando and Selina Siao. Upon arrival, LPAs met with Director, Sandy Cook. Director, Sandy Cook, gave the LPAs a tour of the facility. The following ratios were observed in the facility, there were 117 children to 14 staff members. In the Willems Room, 19 children were being supervised by Preeti Sinha and Valerie Babbe in outdoor play area. The Silverstein Room had 20 children being supervised by Rose Palmer and Wendy Corona in the outdoor play area. The Dean Room had 18 children and was being supervised by Angel Au and Ausra Bunas. The Potter Room had 19 children and was being supervised by Chris Dean and Erin Kohn. The Seuss Room had 19 children being supervised by Christine Smith (Miller) and Alison McCann. The Milne Room had 11 children and was being supervised by Adriana Terzoli and Minjung Song. The Carle Room had 11 children and was being supervised by Cameryn Talpas and Fawn Philbrook. Staff members had the required background clearances and are associated to the facility. The facility self-reported the incident to the department by telephone on 10/28/22 and submitted Unusual Incident Report timely on 11/2/22. The facility reported that a child jumped off a log and hurt his right forearm. At the time of the incident there 15 children being supervised by two staff members in outdoor play area. Based on interviews and observation, it is determined the incident was an accident, no violations will be issued to the facility regarding this incident. Exit interview conducted and report was reviewed with the director, Sandy Cook. A notice of site visit was given and must remain posted for 30 days.
