On 08/25/2025 at 9:00AM, Licensing Program Analyst (LPA) Hanna Lucas made an unannounced case management visit to follow up on a self reported incident that occurred on 08/14/2025 wherein a parent witnessed a child (C1) being pulled by their arm, by a staff member (S1). The incident did not require medical treatment. There were 32 children with six teachers, in 3 classrooms, during today’s visit. At the time of the incident, there was 1 teacher (S1) and 9 children present in Pre K classroom. (S1) was observed by a parent, grabbing (C1)'s arm because the child was not listening. The parent notified the Director, Michelle Pezman. (S1) was placed on administrative leave the same day, 08/14/2025. LPA inspected the facility, interviewed parents and staff members, and collected pertinent documentation. After thorough investigation, a TYPE B deficiency is issued on the attached LIC 9099-D .
