On September 29, 2025, at 2:00 PM, Licensing Program Analysts (LPA), Oscar Picazo conducted an unannounced Annual/Random inspection and met with Site Supervisor, Jonlynn Fuertes. LPA disclosed the purpose of the inspection and was granted entry into the facility. LPA, accompanied by the Site Supervisor, toured the indoor and outdoor of the facility. Also present were Assistant Site Supervisor Laura Calderon and Area Manager Patricia Jaimez. This is a full day program which operates on a year round school year schedule. Days and hours of operation are Monday- Friday from 7:30 AM to 4:30 PM. The following ratios were observed: Room 1 - There was one (1) child (age 2 years) present with one (1) Lead teacher, and one (1) Early Head Start Floaterer. Room 2 - No enrollment. Room 3 - There were 2 children (ages 2 - 2.5 years) present with two (2) Lead teachers. Room 4 - There were 8 children (ages 3 - 5 years) present with one (1) Lead teacher, one (1) Teacher Aide and one (1) Inclusion technician. Site Supervisor stated there are no swimming pools or other bodies of water on the premises. Site Supervisor stated there are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. No poisons, hazardous or unsafe materials were observed during the inspection. Furniture, play area and playground equipment are in good condition, free of sharp, lose or pointed parts. See LIC 809C Continuation... The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All toilets and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. Areas around high climbing equipment, swings and slides have cushioning rubber material to absorb falls. Shade in the outdoor area is provided by a a large tree and shade canopies. The facility is free of flies and insects. Facility has one or more functioning combo carbon monoxide & smoke detectors that meet statutory requirements. The last fire and disaster drills were conducted and documented on 09/16 & 09/18/2025 . The facility provides breakfast, lunch and snack to the children. Menus are posted weekly. Drinking water is available both indoors and outdoors. Staff provide filtered drinking water from a portable jug in all rooms. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Site Supervisor was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Capacity and limitations as specified on the license are being maintained. Children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one (1) teacher, one (1) Associate teacher and/or Aide supervising no more than 4 children each, in rooms one (#1) and two (#3) and 1 a ratio of one (1) teacher and one (1) Associate teacher to 10 in rooms 2 and 4. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility. Site Supervisor Jonalynn Fuertes CRP/First aid expires 03/25/2027. Mandated Reporter Training was taken on 07/02/2025. The name of the facility representative and the fully qualified teacher designated to act in the facility representative’s absence has been reported to the Department. The facility utilizes physical sign in/out sheets and were reviewed by the LPA. The Site Supervisor was reminded that the person who signs the child in and out of the facility shall use their full legal signature and record the time of day. This facility provides Incidental Medical Services – IMS, LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. Site Supervisor was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. No deficiencies were cited during this inspection. An exit interview was conducted and report was reviewed with the the Site Supervisor Jonalynn Fuertes. Appeal Rights (LIC 9058) was provided. Site Supervisor was provided the Notice of Site Visit (LIC 9213) and advised to post and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. LPA observed Notice of Site Visit posted on the interior of the main door.
