What the inspector wrote

On the above date and time, Licensing Program Analyst (LPA), Kelly Gerth conducted an unannounced Case Management visit to follow up on Unusual Incident Report (UIR) that was submitted by the facility and received by Licensing on November 21, 2025. LPA met with Site Director Monique Gapuz to discuss the incident reported. The unusual incident report stated that on 11/19/2025, a child was using the restroom in the left portion of the classroom area and a teacher remained on the right side of the classroom during this time. During the visit, LPA Gerth toured the classroom building and observed the left and right side of the classroom space, and bathroom area. The classroom is used as 2 classroom areas via a moveable, temporary partition that creates 2 spaces. At no time did the child or the teacher exit the actual classroom building and although the child was on the left side of the space, this did not put the classroom out of ratio or leave the child alone. As a precautionary response, however, the CCC did update their internal policy and procedures for staff and discussed the updates at a staff meeting on December 04, 2025. The site Director provided LPA copies of the updated internal policy and procedures a staff meeting sign in/out log and the meeting agenda. Therefore, LPA Gerth determined that the CCC has acted appropriately and not violated any regulations. An exit interview was conducted, and a copy of this report, exit interview and notice of site visit was provided to Director Monique Gapuz. The Facility Representative was reminded that the notice of Site Visit must remain posted for 30 days.
