What the inspector wrote

On the above date and time, Licensing Program Analyst (LPA’s) Kelly Gerth and Gabriela Hernandez arrived at the facility on a case management inspection, to follow-up on an Unusual Incident Report (UIR) received by Community Care Licensing (CCL) on 02/23/2026. LPA’s met with Director Monique Gapuz to discuss the incident. The UIR stated that on 02/19/2026, the Child Care Center (CCC) received information of parent concern that their child may have received an accidental injury or incident while in care the previous day. The parent reported the child had expressed some discomfort and was concerned that something could have occurred while at the facility and was inquiring if any staff had observed any incidents or injury involving the child. After receiving this information, the CCC conducted their own internal investigation, interviewing staff that were present, if they observed any incidents, injuries or heard the child discuss anything relating to the parents concern. The result of the CCC’s internal investigation was that staff were not aware of any incidents occurring at the CCC and did not receive any communication from the child involving any discomfort, injury or incident. Following the internal investigation, the CCC followed up with the parent to inform them that they had not found that anything occurred to the child while in care and the parent thanked the CCC for looking into the matter. The child resumed attendance per usual and no further incidents have been brought to the attention of the CCC. Based on the information obtained during the visit, there appears to be no violations of Title 22 Regulations pertaining to the reported incident. An exit interview was conducted, a copy of this report and Notice of site visit was provided to Director Monique Gapuz who was reminded it must remain posted for 30 days.
