What the inspector wrote

On the above date and time, Licensing Program Analyst (LPA) Kelly Gerth arrived at the Child Care Center (CCC) on a case management inspection, to follow-up on an Unusual Incident Report (UIR) received by Community Care Licensing (CCL) on April 13, 2026. LPA met with Director Monique Gapuz to discuss the incident. The UIR stated that on April 01, 2026 during outdoor activity time, a child fell while playing on the play structures slide. Staff provided first aid and contacted parent for pick up. Per the CCC, the child was taken for medical attention by the parent. The CCC stated the child has not yet returned to care since the date of the incident. During the visit, LPA collected copies of children’s files, Parent Handbook and internal observation notes. Due to the need to gather additional information pertaining to the incident that occurred, LPA will make a subsequent visit to the CCC in response to the unusual incident reported, after further information is gathered and reviewed. An exit interview was conducted, a copy of this report and Notice of site visit was provided to Director Monique Gapuz who was reminded it must remain posted for 30 days.
