On the above date and time, Licensing Program Analyst (LPA) Kelly Gerth arrived at the Child Care Center (CCC) on a case management inspection, to follow-up on an Unusual Incident Report (UIR) received by Community Care Licensing (CCL) on April 13, 2026. LPA met with Director Monique Gapuz to discuss the incident. LPA made an initial case management visit regarding the UIR on April 15, 2026. The UIR stated that on April 01, 2026 during outdoor activity time, a child fell while playing on the play structures slide. Staff provided first aid and contacted parent for pick up. Per the CCC, the child was taken for medical attention by the parent. Based on the information obtained during the visit on April 15, 2026 and additional information collected April 20, 2026 and April 27, 2026, from the CCC, there appears to be no violations of Title 22 Regulations pertaining to the reported incident and LPA determined that the facility took the necessary steps to ensure the safety of all children in care. An exit interview was conducted, a copy of this report and Notice of site visit was provided to Director Monique Gapuz who was reminded it must remain posted for 30 days.
