What Happened
Case management investigation of three self-reported incidents resulted in Type A deficiency citation for unsupervised children on playground. Two other incidents (allergic reaction and fall) determined to not warrant citations.
Observations
- One Type A deficiency: Staff Members #3 and #4 left Child #3 unattended and unsupervised for approximately 3 minutes on playground on 7/9/24
- Child #1 allergic reaction on 7/5/24 determined to have unknown cause; no deficiency
- Child #2 fall injury on 7/15/24 determined to be accident; no deficiency
- $500 civil penalty assessed

101229(a)(1) Responsibility for Providing Care and Supervision. No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation.This requirement is not met as evidenced by: Based on interviews and S3 and S4s own admission, the licensee did not comply with the section cited above in that it was determined that on 7/9/24 there was an Absence of Supervision resulting in C3 was left unsupervised, which poses an immediate health and safety risk for persons in care.
