What Happened
Office meeting to discuss deficiency corrections from prior citations. Facility had been cited for unattended child on playground (Type A) and teacher-child ratio violation (initially Type A, reclassified to Type B). Facility implemented corrective training and supervision procedures.
Observations
- Type A violation 07/16/24: Two staff members left child unattended on playground for approximately 3 minutes on 07/09/2024
- Type A violation 08/07/24 (reclassified to Type B): Teacher-child ratio violation
- Facility provided plan of correction with staff retraining documentation
