What the inspector wrote

On April 30, 2026 at 12:00 p.m., Licensing Program Analyst (LPA) Angela Nguyen and Licensing Program Manager (LPM) Rajani Goudreau conducted an unannounced Annual/ Random Inspection and met with the Licensee, Rebecca Maxwell. LPA disclosed the purpose of the inspection and was granted facility entry by the Licensee. The Licensee, licensee's son, grandson and no day care children were present during the inspection. This facility is a single story, five- bedroom, two bath home. The following areas are used for childcare: living room, kitchen, dining room, bedroom #1, bathroom #1 (hallway), and front yard. The following off-limit areas are bedroom's #2, #3, #4 & #5, backyard and garage. Off limit areas are inaccessible to children by use of key lock doors and child proof locks and a child proof gate separating the day care areas and the off limit areas. Hours of operation: Monday - Sunday from 6:00 a.m. to 11:30 p.m. At 12:41PM, two day care children arrived. The fire extinguisher, smoke detector, and carbon monoxide detector are operational. Hazardous items were observed in a locked cabinet during this inspection. LPA observed children’s toys and play equipment available for children. Licensee uses fenced front yard for outdoor play. Licensee stated there are no bodies of water. LPA observed no bodies of water on the premises during the inspection. Licensee stated there are no weapons in the home. The licensee provided proof of control of property. Licensee’s First Aid and CPR certifications expire in 8/23/2027. Licensee has required immunizations. Mandated Reporter Training expired on 02/03/2027. LPA reminded licensee that Mandated reporter and Pediatric CPR/ First aid expires every 2 years and shall be kept on file at the facility. The facility roster is maintained and was reviewed. The last fire drill was not conducted or documented. Staff records reviewed. Child record review was conducted and determined to be incomplete. LPA obtained a copy of current children facility roster. LPA discussed the facility emergency disaster plan, LIC 311D and safety of children. LPA reminded Licensee of the following: Report suspected child abuse and neglect, maintain children’s records according to regulation, post all required forms. Licensee was reminded that corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers and/or similar equipment are not allowed in daycare. LPA obtained an updated LIC 279 and LIC 855. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. California Code of Regulations, (Title 22, Division 12 & Chapter 3), 1 type B deficiency are being cited on the attached LIC 809-D. Three technical violations were issued. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee, Rebecca Maxwell, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
