On 5/26/2023, at 8:35 am., Licensing Program Analyst (LPA), Vicky Williamson conducted an unannounced annual required Inspection. LPA met with the Licensee, Patricia Guliasi and disclosed the purpose of the inspection. Upon LPA's arrival, there were six (6) children present (two (2) of whom are under 24 months) present during this inspection. At 9:05 am an additional child arrived at the facility. Facility is operating within ratio capacity. Days and hours of operation are Monday - Friday from 6:00 am - 6:00 pm. The indoor and outdoor of this single story, 3 bedrooms, 2 bathroom home was inspected. The following areas are used for childcare: living room, dining room, bedroom 2, bedroom 3, hallway bathroom, backyard patio, and upper backyard play area. Off limits areas include: master bedroom, bathroom 2 (inside of master), laundry room, side yard (right side of patio). They are made inaccessible to day care children by use of door locks, door knob covers and safety gates. Licensee is reminded to maintain visible supervision of children at all times. The fire extinguisher, smoke and carbon monoxide detector meet requirements. Disinfectants, cleaning compounds and medications where inaccessible to children. LPA observed hazardous items accessible to children during the time of inspection. In the patio area where the children play, LPA observed a large black garbage bag on the ground, two water hoses across the patio, exposed wood on the step leading into the children's playhouse in the play area, and a fan with the cord hanging away from the fan accessible to children. Licensee removed the fan, large black bag, and taped the step entering the playhouse during the time of inspection. In the upper backyard play area, LPA observed a piece of wood on the ground, empty bags of soil, aloe vera plants and rose bushes with thorns accessible to children during the time of inspection. Licensee removed the wood and empty soil bags during the time of inspection. Licensee stated she will make the plants inaccessible to children. Licensee is reminded to maintain visible supervision of the children in the upper backyard play area. The licensee has toys, play equipment and materials available. See LIC 809C Continuation... Licensee stated that there are no bodies of water on the premises. LPA observed no bodies of water on the premises during the inspection. Licensee stated there are no weapons in the home. The last fire/disaster drill was conducted and documented on 4/4/2023. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. First Aid and CPR certifications for licensee is valid. Licensee did not have the required immunization records on file during the time of inspection. Mandated Reporter training for licensee is valid. LPA informed licensee to ensure the mandated reporter training is completed once every two years. LPA observed the required documents posted. A sample of children\342\200\231s files were reviewed and determined to be complete. Incidental Medical services (IMS) policy was discussed. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. A Notice of Site Visit was given and must remain posted for 30 days from today\342\200\231s date. LPA observed Notice of Site Visit posted to bulletin board in the entry area of the facility. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
