What the inspector wrote

On March 3, 2022 Licensing Program Analyst (LPA), Dana Stevens conducted an unannounced Annual Required Inspection and met with the Licensee, Gilda Zandi. LPA disclosed the purpose of the inspection and was granted entry into the facility by the Licensee. Four children, ages 16 months, 4,4, and 4 were present with Licensee during this inspection. This facility is a two story, 4 bedroom, 2 1/2 bathroom house. The following areas are used for child care :\302\240Kitchen, family room, dining room, living room and downstairs bathroom. Off limits areas are the entire upstairs and garage. These areas are made inaccessible through use of a safety gate and door locks. The fire extinguisher, smoke detector, and carbon monoxide detector met requirements.\302\240 All haza rdous items were inaccessible to children.\302\240The licensee has toys, play equipment and materials available.\302\240 The home has a fenced yard available for outdoor activities.\302\240 No bodies of water observed on the premises during the inspection. Licensee stated there are no weapons in the home. Individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions.\302\240 Licensee\342\200\231s First Aid and CPR certifications expire on 07/2023 .\302\240 Licensee has required immunization.\302\240 Licensee completed Mandated Reporter Training on 10/2021. Facility roster is maintained and was reviewed.\302\240 The last fire and disaster dril ls were conducted and documented on March 1, 2022. There is one crib or play yard for each infant who is unable to climb out of the crib or play yard. Cribs or play yards are free from all loose articles and objects. The provider physically checks on sleeping infants every 15 minutes. LPA provided and discussed the following:\302\240 Report suspected child abuse and neglect, maintain children\342\200\231s records according to regulation, post all required forms, and ensure that all adults residing or working in the home have criminal background clearances or exemptions.\302\240 Licensee was reminded that corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers and/or similar equipment are not allowed in daycare. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violate d. Incidental Medical services (IMS) policy was discussed.\302\240 For IMS information see Evaluator Manual \342\200\223 Regulation Interpretations and Procedures for Family Child Care Homes Section 102417.\302\240 When any IMS is provided, a Plan for Providing IMS must be submitted to the Department.\302\240 The following information regarding ADA was provided:\302\240 US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication:\302\240 Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm . No deficiencies cited An exit interview was conducted with the licensee.\302\240 The licensee was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights. LPA provided notice of site visit and it must be posted for 30 days.
