On October 17, 2025, at 1:30 PM, Licensing Program Analyst (LPA) Hector Canton arrived unannounced at the licensed Family Child Care Home operated by Lia Woldu to conduct the required annual inspection. LPA identified themselves, stated the purpose of the visit, and was granted entry. Thirteen daycare children are present during today\342\200\231s inspection: nine preschool age, one infant, and three school age. Upon arrival, LPA met with assistant and spouse Fassila Tsegaye. Proper ratio and capacity observed during today\342\200\231s visit. The one-story five-bedroom three-bathroom single family home was toured to evaluate compliance with Title 22 regulations and to ensure the health and safety of children in care. The child care areas include the nursery room, main kitchen, living room, bathroom 1, and currently using kitchen 2 and the second living room for child care during home renovations. Areas made off-limits include bedroom 1-5, bathroom 2, and bathroom 3 were made inaccessible using door knob covers. Adequate heating, lighting, and ventilation were observed throughout the licensed areas of the home. The licensee is currently renovating parts of the home, LPA advised that any changes to the physical plant of the facility will require a new facility sketch and any changes to licensed child care areas will require approval prior to use. A working telephone was available for use in case of emergency. Age-appropriate toys, furnishings, and play equipment were observed to be in good condition and free of observable hazards. LPA did not observe any bodies of water on the premises. The licensee confirmed that no firearms or other weapons are kept in the home. LPA observed that detergents, cleaning compounds, medications, and other potentially hazardous items shall be stored inaccessible to children. LPA observed the fire extinguisher to be fully charged and operational. Smoke detectors and carbon monoxide detectors were tested and found to be operable at the time of inspection. Emergency exit routes were clear of obstructions. LPA observed the outdoor area to be fully fenced, due to the renovations, there are some building materials in the yard that are made inaccessible using child fences. The licensee understands that supervision is always required when children are outdoors and ensure proper inaccessibility to any potential hazards. A designated isolation area is available should a child become ill during the day. There were no jumpers or other prohibited equipment observed. All equipment used in care was used only as intended by the manufacturer. Children\342\200\231s records were reviewed. LPA verified that emergency contact information, immunization records, and required notices were available and complete. A current facility roster was maintained and provided upon request. The licensee maintains documentation of emergency disaster drills, and the most recent drill was conducted in July 1, 2025. All required postings, including the facility license and parents\342\200\231 rights notification, were observed in a location accessible to parents and guardians. LPA reviewed staff and adult resident records. All adults living or working in the home have been fingerprint-cleared and associated with the facility. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. The licensee did not have a verifiable Pediatric CPR and First Aid certification available during today\342\200\231s inspection, however, states that it was taken last year (2024). Licensee, Lia Waldu, also does not have a verifiable CPR/First Aid certificate available for the department to review for any personnel at the time of the inspection. The licensee is exempt from completing the required Mandated Reporter Training as their primary language is Tigringa and the training is only available in English and Spanish at this time. LPA verified that the licensee has met immunization requirements. LPA observed infants in care and conducted a review of safe sleep practices. Each infant had their own designated crib or play yard that was free of loose articles, pillows, or soft bedding. Bumper pads were not in use, and there were no objects hanging from or attached to any of the sleep spaces. The licensee confirmed that infants were placed on their backs to sleep and not swaddled. The licensee confirmed that infants are actively supervised while sleeping and that a 15-minute sleep log is maintained and available for review. Individual Infant Sleeping Plans (LIC 9227) were on file for each infant under 12 months of age. LPA reviewed this documentation and found it to be complete. Car seats were not used for sleeping purposes and were only used for transportation. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. The facility does not currently provide Incidental Medical Services (IMS). LPA discussed the IMS policy and explained that when IMS is provided, a Plan for Providing IMS must be submitted to the Department. The licensee was provided with guidance available in Provider Information Notice PIN 22-02-CCP. Additional ADA resources were discussed, including the U.S. Department of Justice ADA Information Line at (800) 514-0301 (voice) or (800) 514-0383 (TTY), and the publication \342\200\234Commonly Asked Questions About Child Care Centers and the ADA,\342\200\235 which is available online at https://www.ada.gov/resources/child-care-centers/ . Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. LPA also provided the licensee with the Child Care Advocates Program contact email at childcareadvocatesprogram@dss.ca.gov, and advised the licensee that questions regarding licensing requirements may be directed to the Child Care Duty Line at (619) 767-2248. Pursuant to Title 22, a type B violation was cited during today\342\200\231s inspection and the deficiency is documented on the LIC 809D. An exit interview was conducted with the licensee, and this report was reviewed in its entirety. A copy of the report was provided. Appeal rights were also reviewed and issued. A Notice of Site Visit was issued and must remain posted for 30 consecutive days. The licensee, Lia Waldu, confirmed that there are no registered sex offenders residing at the facility address, and LPA completed the Registered Sex Offender (RSO) profile in FAS.
