What the inspector wrote

Due to conflicting statements and the lack of supporting evidence, LPA is unable to determine whether or not the allegation occurred. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. No deficiencies were cited. An exit interview was conducted with the Licensee and a copy of this report, along with Licensee Rights (LIC 9058 01/16), were provided. LPA provided a Notice of Site Visit (LIC 9213), which must remain posted for 30 days. LPA observed that the LIC 9213 was posted during the inspection.
