What the inspector wrote

*THIS IS AN AMENDED VERSION OF A PREVIOUSLY WRITTEN REPORT *On February 18, 2026 at 12:30PM, Licensing Program Analyst (LPA), Adrian Castellon conducted an unannounced complaint inspection to deliver the finding for the above listed allegation. LPA met with Licensee, Alejandra Perez, and advised licensee of the purpose of the inspection and conducted a tour of the home. There were eight (8) children in care with three (3) staff present. It was alleged that licensee caused injuries to an infant in care. The investigation was conducted by Community Care Licensing Division Investigations Branch (IB), Investigator, Annette Renquist. During the course of the investigation, interviews were conducted with the Licensee, day care parents, staff, and representatives from outside agencies. Facility records and medical records were reviewed.Due to conflicting information obtained throughout the course of the investigation and no other witnesses to the alleged injury, Investigator Renquist was unable to determine whether or not the allegation occurred. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Exit interview conducted and report was reviewed with the Licensee, Alejandra Perez. A Notice of Site visit was given and must remain posted for 30 days.
