Page 2 Although narratives from interviews conducted did vary slightly, interviews did corroborate that the method of discipline used at the time of the incident was not an appropriate method of discipline. LPA found that the method used was a violation of personal rights of children in care. Therefore, based on LPA’s observations, interviews conducted and documents reviewed, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. Regarding the allegation Licensee does not treat daycare children with dignity and respect. During the investigation, LPA conducted interviews and made observations during the visit to the home daycare on 12/05/2025. Although the language used by the Licensee may have the intent of redirection for children’s health and safety, some verbiage used at times and delivery method was found to be in violation of children’s personal rights and inappropriate to use for redirection and/or addressing children in care. Based on LPA’s observations, interviews conducted and documents reviewed, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. See 809 D Page An exit interview was conducted, and this report was reviewed with Licensee Syeda Imtiaz. Appeal rights were discussed and provided during the exit interview. A notice of site visit was given and licensee understands this must remain posted for 30 days.

This requirement was not met as evidenced by; Based on observation and record review the licensee did not comply with the section cited above in 1/1 infants napping in care did not have a sleep log on file which poses a potential health, safety or personal rights risk to persons in care.

    102423(a)(1) Each child receiving services from a family childcare home shall have certain rights that shall not be waived or abridged by the licensee regardless of consent or authorization from the child's authorized representative. These rights include, but are not limited to, the following: (1) To be treated with dignity in his/her personal relationship with staff and other persons. This requirement was not met as evidenced by: Based on observation and record review the licensee did not comply with the section cited above when language and method used by Licensee, intended as redirection on at least one occasion, was not appropriate and violated children’s personal rights, which poses a potential health, safety or personal rights risk to persons in care.
