Due to conflicting information and a lack of supporting evidence obtained throughout the course of the investigation from observations and interviews conducted, the allegations are determined to be unsubstantiated. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated. An exit interview was conducted with the licensee. Appeal Rights (LIC 9098 01/16), Notice of Site Visit (LIC 9213) along with a copy of this report was discussed and provided to licensee. LPA informed licensee Notice of Site visit shall be posted for 30 days from today\342\200\231s date.
