What the inspector wrote

On October 10, 2025 at 9:45 p.m., Licensing Program Analyst (LPA) Angela Nguyen conducted an unannounced Annual/ Random Inspection and met with the Licensee, Taheria Melvin. LPA disclosed the purpose of the inspection and was granted facility entry by the Licensee. The Licensee, her 2 minor children and 4 day care children were present during the inspection. This facility is a one 1 story, 4 bedroom, 2 bathroom house. Licensee accompanied LPA inside and out of the facility during this inspection. The following areas used for childcare are the living room, Bedroom 1-3, kitchen, and hallway bathroom. Off limits areas are the master bedroom and bathroom, and the garage which are made inaccessible through use of child proof lock on door. The fire extinguisher, smoke detector, and carbon monoxide detector are operational. Hazardous items were observed in a high cabinet inaccessible to children. LPA observed children\342\200\231s toys and play equipment available for children. Licensee uses fenced backyard for outdoor play. There is a fireplace with a safety screen in the home. Licensee stated there is a fenced in-ground pool on the premises. LPA observed a 60 inch tall mesh fencing with no gap from the ground with a self- closing- self latching pool gate that swings away from the pool. LPA observed the lock on the pool gate. LPA inspected the pool alarm, 12 feet pool pole with body hook, and 22 inch life ring that is approved by the U.S. Coast Guard to meet AB2866 requirements. Licensee stated there are no weapons or ammunition in the home. Licensee\342\200\231s First Aid and CPR certifications expire in 3/27/2027. Licensee has required immunizations. Mandated Reporter Training expired on 6/27/2025. J.Melvin's Mandated Reporter training expires on 4/2027. CPR/First aid expires in 3/21/2027. LPA reminded licensee that Mandated reporter and Pediatric CPR/ First aid expires every 2 years and shall be kept on file at the facility. The facility roster is maintained and was reviewed. The last fire drill and disaster drill was conducted and documented on 6/7/2025. Staff records reviewed and determined to be complete. Child record review was conducted and determined to be complete. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. There is one crib or play yard for each infant who is unable to climb out of the crib or play yard. Cribs or play yards are free from all loose articles and objects. Licensee physically checks on sleeping infants up to 24 months of age every 15 minutes. An Individual Infant Sleeping Plan [LIC 9227 (3/20)] shall be maintained for each infant up to 12 months of age. Licensee places infants up to 12 months of age on their backs for sleeping. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee stated Incidental Medical Services (IMS) policy was reviewed with Licensee. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA obtained a copy of current children facility roster. Licensee and LPA discussed the facility emergency disaster plan, LIC 311D and safety of children. LPA reminded Licensee of the following: Report suspected child abuse and neglect, maintain children\342\200\231s records according to regulation, post all required forms. Licensee was reminded that corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers and/or similar equipment are not allowed in daycare. Licensee was informed of the MyChildCarePlan.org website; California\342\200\231s consumer education website for locating child care services. This website will include all licensed child care providers statewide. During the exit interview, Taheria Melvin, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile. One Type B , California Code of Regulations, (Title 22, Division 12 & Chapter 3), are being cited on the attached LIC 809-D. A Notice of Site Visit (LIC 9213) was given to Licensee, Taheria Melvin and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted and the report was reviewed with Licensee, Taheria Melvin.
