What the inspector wrote

LPA informed facility Licensee, Mounia Chemsi, that this report dated 10/21/2022 documents one (1) Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA informed Licensee to provide a copy of this licensing report dated 10/21/2022 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with Licensee, Mounia Chemsi. A copy of this report, along with Appeal Rights (LIC9058 03/22), were provided. A notice of site visit was given and must remain posted for 30 days. LPA observed that the notice of site visit was posted during the inspection. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Safety precautions shall include but not limited to: (3) Where children are less than five years old are in care, stairs shall be fenced or barricaded.This requirement is not met as evidenced by: Based on LPA observations, the staircase in the living room is not barricaded, which poses a potential health, safety or personal rights risk to persons in care.

    ARTICLE 2. Administration of Child Day Care Licensing

    Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request. This requirement is not met as a facility roster is not available for review today. This poses a potential health and safety risk to clients in care.
