What the inspector wrote

On September 24, 2025, at 3:45 p.m., Licensing Program Analyst (LPA) Angela Nguyen conducted an unannounced case management inspection. LPA met with Licensee, Michele Galvez and was granted entry into the facility. LPA inspected the inside and outside of the home. There were 6 day care children, helper Citlaly Ramos and Licensee's 4 children were present during the inspection. Day care areas: Living room, Hall bathroom, kitchen and backyard. Hours of operation: Monday - Friday 5:00 a.m.- 10:00 p.m. On April 29, 2025 Licensee submitted a request to go on inactive status from May 1- November 1, 2025. On September 12, 2025, Licensee submitted a written request to return to active status and to inspect the outdoor play area. During the inspection, LPA observed 13 cans of paints in the hallway accessible to children. LPA obtained an updated LIC 279 Family child care Application. One Type B California Code of Regulations, (Title 22, Division 12 & Chapter 3), are being cited on the attached LIC 809-D. A Notice of Site Visit (LIC 9213) was given to Licensee, Michele Galvez and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted and the report was reviewed with Licensee, Michele Galvez.
