What the inspector wrote

On March 12, 2026 at 12:40 PM, Licensing Program Analysts, (LPAs), Angela Nguyen and Michelle Hood conducted an unannounced Plan of Correction (POC) inspection at the facility. Purpose of this inspection is to ensure citation issued during a Case Management (CM) inspection dated February 26, 2026 was corrected. Upon arrival, LPAs met with Missonlye Conserve. Conserve stated that she contacted her sister and allowed us in the home. At 12:45, Multilingual Language services was contacted for Hatian Creole translation (operator# 98471797). LPA\342\200\231s observed 3 children in care. One of those children is an infant. At 12:55 PM, Licensee, Vanessa Payant, Pikenson Conserve, their minor daughter and one day care child arrived at the facility. The original Plan of Correction (POC) was due on March 5, 2026. The licensee failed to correct the cited deficiency. During an inspection on February 26, 2026, the licensee was unable to provide the Department with the original lease agreement as previously requested. The document was required to be submitted to the Department by March, 5 2026. Due to the licensee\342\200\231s failure to correct the deficiency by the required due date, a civil penalty is being assessed. See LIC 421FC. A notice of site visit was given and must remain posted for 30 days. An exit interview was conducted, and the report was reviewed with the licensee, Vanessa Payant. The licensee was provided with a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights.
