What the inspector wrote

On 5/30/2025, at 8:30 AM, Licensing Program Analysts (LPA) Oscar Picazo conducted an unannounced case management inspection to follow up on a self reported incident. LPA met with Licensee, Leticia Gonzalez. Upon arrival, LPA was greeted and granted entry into the facility by Licensee. LPA discussed the purpose of the visit and was led on a tour of the facility indoor and outdoor. Also present during this visit was Licensees assistant, Maria P Hernandez (S1) and 10 day care children, two of which are under 24 months of age. On 05/05/2025, Licensee reported an incident regarding a possible lack of supervision involving 2 day care children, child #1 (C1) & child #2 (C2), where C1 sustained an injury to the back of the head that possibly required medical attention. Per the Licensee, the alleged incident occurred on 05/05/2025, at about 9:40 AM, in the rear day care room. Interviews were conducted with the Licensee, staff (S1), and day care children, including C1 & C2. LPA obtained a copy of the facility roster and sign in/ sign out sheets. LPA inspected the rear day care room to determine that it is safe and age appropriate. Licensee reported that she, her assistant and 10 children were in the rear day care room during the time of the incident. Licensee stated that C1 & C2 were dancing at the end of circle time as usual. Licensee observed C2 go to hug C1 while he was dancing which caused C1 to lose his balance and fall backward and hit the back of his head on the edge of a cubby shelf. Licensee stated that she was in the room during the time of the incident; however she was on the opposite side of the room with a group of five (5) children and was too far to react in time to break the fall. Licensee stated that C1\342\200\231s authorized representative was contacted immediately and the child was picked up within the hour. See Continuation 812C... Staff #1 (S1) stated that she was positioned on the side of the room that C1 and C2 were dancing on during the time of the incident with 5 children; however C1 was at least 3 arm lengths away from her and she was unable to break C1\342\200\231s fall. Based on staff and day care children interviews it was determined that the rear child care room is safe, age appropriate and there was appropriate staff supervision in the room during the time of the incident. The incident is determined to be an accident.
