What the inspector wrote
On 3/11/2025, at 9:15am, Licensing Program Analysts (LPA), Vicky Williamson and Jacqueline Macias conducted an unannounced annual random inspection and inspected facility for room addition. LPA met with Licensee, Maryam Hoseini and Gholam Gholami and discussed the purpose of the inspection. At 10:00am LPA Jacqueline Macias and Licensee's Adult Daughter, Zahra Gholami arrived to the facility. Zahra Gholami assisted with translation due to licensee's primary language is Farsi. There were two (2) daycare children present, one (1) of whom is an infant. Days and hours of operation are Monday through Sunday, 23 hours per day.
LPAs accompanied by Licensees toured the indoor and outdoor of the home. This is a tri level home with seven (7) bedrooms and eight (8) bathrooms. The following areas are used for daycare: second level which includes the living room, kitchen, bedroom, bathroom (inside of bedroom), bathroom (leading from living room). Off-limits areas includes first level, unit one (1 bedroom and 1 bathroom), second level, unit two leading out from sunroom (1 bedroom and 1 bathroom), second level with entrance on back of home ( kitchen, living room, 1 bedroom and 1 bathroom), third level (living room, kitchen, dining room, 3 bedrooms, 3 bathrooms, balcony and laundry room), the garage, detached room behind the home (2 bedroom and 1 bath), and upper back yard area and fenced garden area. Off limit areas are made inaccessible to children through the use of door locks and doorknob covers. Licensees stated that a nearby park is used for outdoor activities. LPAs observed the stairway leading to the second level was not barricaded.
LPA advised the Licensee that prior to making alterations or additions to the home or grounds, the Licensee shall notify the Department of the proposed change. Licensees will provide a detail sketch to the SDRO.
The fire extinguisher, smoke and carbon monoxide detectors meet requirements and are operational. All poisons, detergents, cleaning compounds, medications and other hazardous items in the home are made inaccessible to children through latches, locks, and/or placed on high surfaces. There are three (3) fireplaces in the home. One fireplace located on the second level and two fireplaces located on the third level of the home. Licensee has children's toys, play equipment and books available. Licensee has working cell phone. The last fire drill was conducted and documented on 10/16/2024 and earthquake drill on 10/24/2024.
Licensees stated that there is an in-ground swimming pool and in-ground jacuzzi on the premises. LPAs observed an in-ground swimming pool and jacuzzi with a 5 feet high fencing enclosing the bodies of water, the fence does not obscure the pool from view. There are four entrance gates that swings away from the pool, are self-closing and do not have key lockable devices that meet pool safety requirements. LPAs observed a one pool alarm, a life ring and pool rescue pole with a body hook that meet new pool safety requirements. There was no pool alarm for the jacuzzi. Daily safety inspection log was not available for review during time of inspection.
Licensees stated there is one firearms on the premises that is located in an off limits room. LPAs verified that the firearm and ammunition is stored separately.
Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
Pediatric CPR/First Aid certification for licensees Maryam Hoseini is valid through 9/11/2026 for Gholam Gholami through 5/22/2025. Licensee has required immunization records on file. Mandated Reporter training certification for licensees expired 10/2024. LPAs informed licensees to ensure the mandated reporter training is completed once every two years. LPAs observed the required documents posted. A sample of children\342\200\231s files were reviewed and were determined to be incomplete for Child #1 (C1). C1 did not have children's records or immunization records available for review during time of inspection.

LPA and licensees discussed and reviewed the following: Report suspected child abuse and neglect, form LIC 311D, maintaining children\342\200\231s records according to regulation, posting required forms. Licensee is reminded that corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers and/or similar equipment are not allowed in daycare. Licensees were also provided information regarding SIDS, Lead Exposure and Shaken Baby Syndrome.

Licensee completed the Health and Safety Issues Family Child Care Regulation 102417 form during time of inspection.
Per California Code of Regulations, Title 22, Division 12, Chapter 3, Two (2) Type A and Five (5) Type B Deficiencies are being cited on the attached LIC 809D.
LPA
informed Licensee, Maryam Hoseini that this report dated 3/11/2025 documents two (2) Type A citation, which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA informed Licensee, Maryam Hoseini to provide a copy of this licensing report dated 3/11/2025 that documents two (2) Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. LPA provided Licensee, Maryam Hoseini, with form LIC 9224.
During the exit interview, the Licensee, Maryam Hoseini, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
A Notice of Site Visit (LIC 9213) was given to Licensee, Maryam Hoseini and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted, and the report was reviewed with the Licensee, Maryam Hoseini and Zahra Gholami translated report in Farsi.
