Inspector’s narrative

What the inspector wrote

On 04/08/2025 at 12:30PM, Licensing Program Analyst (LPA) Saul Zazueta conducted an unannounced case management inspection regarding a self-reported incident wherein two siblings brought a bullet casing and a live bullet to the daycare. LPA met with Licensee, Alexandra Carrillo. LPA discussed the purpose of the inspection and toured the facility. During the inspection there were three (3) infants, one (1) preschool child, and licensee present. Licensee stated that Child #1 (C1) found an empty bullet casing on the ground in the living room floor as day care children were watching tv, waiting to be picked up. Licensee immediately picked up the casing and Child #2 (C2) proclaimed that it was his. Licensee asked C2 if they knew what it was and said "yes it's a bullet". Licensee then asked C2's older sibling, Child #3 (C3), if they had anymore and they said "yes, in their backpack". Children both stated that they found them when they went off-roading with their dad. Both children were terminated the same day. Licensee immediately made a group chat with the step dad, mom, and biological dad and notified them of termination. Licensee verbally let the other parents know of the incident. Licensee plans on adding a zero tolerance policy regarding guns and ammo to their contract. Licensee removed the bullet and casing from the facility. Based on assessment, it was determined that the licensee addressed the incident appropriately and reporting requirements were met. No deficiencies were cited during today\342\200\231s inspection. Exit interview conducted and report was reviewed with Licensee, Alexandra Carrillo. A Notice of Site Visit was given and must remain posted for 30 days. This is an amended version of the original report created 04/08/25.
