What the inspector wrote

On June 4, 2025, at 10:32 a.m., Licensing Program Analyst (LPA), Angela Nguyen conducted an unannounced Plan of Correction (POC) inspection. LPA contacted Licensee at 10:35 a.m. Licensee arrived at the facility at 11:07 a.m. LPA met with Licensee, Wister Gaetan and was led on a tour of the facility. The purpose of this inspection is to ensure that citations issued during the Annual inspection on 5/7/2025 were corrected. Tertulia Louisius, Licensee's son and four (4) day care children were present during this inspection. LPA verified that the following correction was completed: All adults working or residing in the home has received criminal record clearances. All detergents and cleaning compounds are made inaccessible to children. -Children's Records for Child #1, Child #2, Child #3, Child #4 were reviewed during the time of inspection and were determined to be complete. A Notice of Site Visit (LIC 9213) was given to Licensee, Wister Gaetan and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. A Large License was issued effective 6/4/2025. Licensee requested to pick up physical license at SDRO. An exit interview was conducted and the report was reviewed with Licensee, Wister Gaetan.
