What the inspector wrote

On March 3, 2026, at 3:40PM, Licensing Program Analyst (LPA), Vicky Williamson conducted an unannounced Annual Random inspection and inspected the facility for a change of capacity. LPA met with Licensee Karen Duenas and discussed the purpose of the inspection. Also present is Licensee’s fiancee Ignacio Castro Aguirre, licensee's minor child and cousin Camila Avila Delgado. There were 4 daycare children present. Days and hours of operation are Monday through Friday, 5:00AM to 10:00PM. LPA accompanied by licensee toured the single level, 3 bedrooms, 2 bathrooms home. The following areas are used for childcare: playroom, kitchen, dining room, bathroom 2 (hallway), and the backyard. Off limit areas are the living room, bedroom 1, bedroom 2, bedroom 3, bathroom 1 (located between bedroom 1 and bedroom 2), barricaded portion of side yard on the left side of home and the garage. Off limit areas are made inaccessible to children by use of doorknob covers, safety gates, and fencing. Licensee stated that the backyard is used for outdoor activities. LPA reminded licensee that children should always be supervised during outdoor activities. Licensee updated the facility sketch during time of inspection. LPA advised the licensee that prior to making alterations or additions to the home or grounds, the licensee shall notify the Department of the proposed change. The fire extinguisher, smoke and carbon monoxide detectors meet requirements and are operational. All poisons, detergents, cleaning compounds, medications and other hazardous items in the home are made inaccessible to children through latches, locks, and/or placed on high surfaces. Licensee has children's toys, play equipment and books available. Licensee has a working cell phone. LIC 809C Continuation… Licensee stated there are no bodies of water on the premises. LPA observed no bodies of water on the premises during time of inspection. Licensee stated there are no firearms, other weapons, or ammunition in the home. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Pediatric CPR andFirst Aid certification for licensee is valid through May 2027. The licensee has the required immunization records on file. Mandated Reporter training certification for licensee is valid through January 13, 2028. LPA informed licensee to ensure the mandated reporter training is completed once every two years. LPA observed the required documents posted. A sample of children’s files were reviewed and were determined to be incomplete. Documentation of 15 minute sleep checks for Child #1 (C1) were not available for review during time of inspection. The last fire drill was conducted and documented on March 17, 2025. Licensee is reminded that fire drills must be conducted once every 6 months. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. See LIC 809C Continuation... Entrance Checklist was provided to the applicant. Licensee is reminded that corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers and/or similar equipment are not allowed in daycare. The licensee was also provided with information regarding SIDS, Lead Exposure and Shaken Baby Syndrome. LPA discussed and provided Licensee with the following: childcare advocate email address: childcareadvocatesprogram@dss.ca.gov . Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. California Code of Regulations, Title 22, Division 12, Chapter 3, deficiencies are being cited on the attached LIC 809D. See LIC 809C Continuation... A fire clearance was granted for 14 children on February 26, 2026. LPA and applicant discussed the maximum number of children for whom care shall be provided when there is an assistant in the home, including children under age 10 who live in the licensee's home shall be either: Twelve (12) children with no more than four of whom may be infants and 8 older children over the age of 2 years or Fourteen (14) children, with no more than three infants (birth to 24 months) and at least two of the children with 1 child enrolled in kindergarten and 1 child at least six years of age with landlord consent (LIC 9149). Because the licensee rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC 9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149). A Large Family Child Care Home license may be issued upon final file review and completion of the following: - Conduct and document current fire and disaster drill. - Complete documentation of 15 - minute sleep checks for C1 for the period of March 4 - March 10, 2026. During the exit interview, the Licensee Karen Duenas, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
