What the inspector wrote

On April 20, 2026, at 2:31 PM, Licensing Program Analyst (LPA) Vicky Williamson conducted an unannounced Case Management inspection to cite for deficiencies observed during another inspection at the facility. LPA met with Licensee Karen Duenas and discussed the purpose of the inspection. Also present is licensee’s assistant Camila Avila Delgado. There were seven daycare children present, four of whom are infants. Upon entering the facility, LPA observed Child #1 (C1) napping in a play yard located in the dining room area of the facility. LPA observed C1 with a pacifier in their mouth, laying on a grey pillow with a folded blue blanket on the right side of their body, a red blanket covered a portion of C1's face and mouth. LPA also observed two toys in the play yard near C1's feet. Licensee immediately removed the blankets, the pillow and the toys from the play yard. LPA and licensee discussed the safe sleep regulation and licensee was provided a copy of California Code of Regulation (Title 22, Division 12, Chapter 3), Section 102425 Infant Safe Sleep. LPA Vicky Williamson informed licensee Karen Duenas that this report dated April 20, 2026, documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPA Vicky Williamson informed the licensee Karen Duenas to provide a copy of this licensing report dated April 20, 2026 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Licensee Karen Duenas. Licensee had to leave the facility at 3:05 PM. Facility Report was left a the facility with licensee’s assistant Camila Avila Delgado.
