On 5/28/26, LPA (Licensing Program Analyst) Saraliz Velando, conducted an unannounced follow up of a self-reported incident that occurred on 4/30/26. The incident was regarding a 6 year old child (Child #1) that reported to their parent that he was hit in the face with a ball. LPA spoke with Staff #1 (S1) who was supervising and playing with the group of TK and Kindergarten aged children. S1 stated that no children reported any injuries during the game. S1 also stated that C1 did not report or complain about any injuries to his face from children or staff. LPA also spoke with Staff #2 (S2) that stated C1 reported that he was hit by a ball on the face as they walked together to the pick up area and informed S3 via Walkie to let the parent know what happened, due to injury report not ready at this time. S1 and S2 both stated there were no complaints or injuries reported by the children. Parent was notified verbally at time of pickup. The following day, the parent stated her concern regarding C1 told her he was hit in face with a ball 3 times on purpose by S1. The report was elevated to the Site Coordinator, Saira Salazar and the Area Manager, Jennifer Rodriguez. During staff interviews, there was no corroborating evidence to prove that C1 was hit or injured in any way by a child or staff member. The ratio was 4 staff to 32 children at the time of the alleged incident, adequate supervision was in place, and staff responded appropriately. The facility reported the incident timely. The games are also age-appropriate for the children. No deficiencies were cited. A notice of site visit was posted and must remain for 30 days.
