This is an Amended Document from the original on 8/13/25. On 8/13/25, Licensing Program Analyst (LPA), Saraliz Velando conducted an unannounced Annual Inspection with a Case Management inspection for Increase in Capacity and Adding Rooms. Licensee\342\200\231s total capacity will increase from 90 to 140 School Age children. Upon arrival, LPA met with Area Manager, April Smith. Licensee is adding rooms #304, #305, and Multipurpose Room. Facility\342\200\231s hours of operation are Monday to Friday from 7:00am to 6:00pm. There were no children present during today\342\200\231s inspection. LPA inspected the rooms to be used. Furniture and age-appropriate equipment is in good condition. Rooms have adequate heating, lighting, and ventilation. Floors appear to be clean and safe. Safe drinking water is readily accessible. Bathrooms are maintained with operational toilets and faucets. Paper towels and toilet paper are available. Bathrooms are lighted and have ventilation. Facility serves only snacks in the outdoor picnic area. Food service area consists of a refrigerator, which is clean and free of hazards and located in Room #304. Facility representative was advised to ensure that all snacks are stored in sealed and dated containers. Snack menu is posted. Cleaning supplies are kept separate from food and are inaccessible to children. Facility representative was reminded to ensure that all storage containers for solid waste have tight-fitting covers. Storage areas for poisons are locked. Facility representative stated there are no firearms or other weapons on the premises. All foods/beverages are stored in covered containers or individually wrapped. The facility appears to be free of insects and rodents. There is an operational carbon monoxide detector at the facility. Facility has 2A10BC fire extinguisher that meets regulations. Fire clearance was granted on 10/23/24. Outdoor play areas are a fenced and age appropriate for school age children. The surface of the outdoor activity space is maintained in safe condition and free of hazards. Facility provides basketballs, hand balls, jump ropes, and other equipment. There are no bodies of water at this facility. There is adequate shade provided outside in the picnic area. Area Manager, April Smith was reminded that any changes to the facility must be reported to and approved by the Department. A license for a total of 140 School Age children (TK-12 years old) in Rooms #304, #305 & Multipurpose Room will be granted effective today. A notice of site visit was given to the facility representative and must remain posted for 30 days. Exit interview was conducted and report was reviewed with Area Manager, April Smith. A copy of this report was also provided.
