What the inspector wrote

On 4/8/2025 at 1:30PM, Licensing Program Analyst (LPA) Hanna Lucas and Licensing Program Manager (LPM) Joelle Redding, made an unannounced case management visit to follow up on a self-reported incident that occurred on 3/17/25 wherein a child (C1) was injured on the playground. There were 71 children with six teachers (S1 through S6) during today’s visit. LPA and LPM toured the playground, inspected the cushioning, observed children playing on the playground and play structure and interviewed all six staff present (S1 thru S6). Appropriate supervision was in place and the cushioning around the play structure was sufficient with the addition of gym mats around the play structure. At the time of incident, there were 3 teachers and 28 children. Although the incident wasn’t directly observed, S1 was standing close by and was able to immediately respond to the child. Ratios were met and supervision was in place. Padding had been placed around the structure and the equipment was age appropriate. Additional gym mats have been added to prevent a repeat incident. It does not appear that the actions or inactions of the facility or facility staff contributed to the injury. No deficiencies cited today.
