What the inspector wrote

On 3/26/2026 at 11:00 A.M, LPA Sharon Mendez and LPA Jennifer Irving made an unannounced visit to follow up on a self-reported incident, received on 3/9/2026. Upon entry, LPAs met with Director, Sarah Clariza and explained the purpose of the visit. A tour of the facility was conducted. There were 75 Children with 9 Staff, facility was within ratio and capacity. During today’s visit, LPAs conducted interviews with Staff, reviewed pertinent files, and received a copy of the children’s roster, personnel report, sign in/out sheet and ouch report. Facility reports that on 3/09/2026 C1 was injured while cleaning up in the kitchen/dramatic play area resulting in medical attention being required. S1 called S2 to take cover the class while S1 took C1 to S3 to assist with injury. S3 notified parent and C1 was pick up at 11:36pm. C1 returned to school the next day. LPAs review the play area where incident occurred and it does not show any immediate hazards accessible. All toys and equipment/furniture are in good repair. No deficiencies cited at this time. Exit interview conducted and report was reviewed with the Director Sarah Clariza. A notice of site visit was given and must remain posted for 30 days.
