What the inspector wrote

On 12/7/2022 @ 2:30 p.m., Licensing Program Analyst (LPA), Joelle Redding, made an unannounced visit to follow up on a self-reported incident that occurred on 11/18/2022. During this visit, LPA spoke with Ms. Gutierrez who is currently supervising and was present on the day of the event. Based on the information provided, lack of direct supervision by Staff #1 resulted in children engaging in an inappropriate game. Staff #1 member is longer employed with the YMCA. A Type B deficiency will be issued on the accompanying LIC 809D NOTICE OF SITE VISIT WAS GIVEN AND WILL REMAIN POSTED FOR 30 DAYS. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
