What the inspector wrote

Based on conflicting statements and limited evidence, LPA was unable to substantiate the allegation that staff failed to take precautions to prevent the spread of illness. While the incident may have occurred, there is not sufficient evidence to support the allegation. Therefore, the allegation is deemed unsubstantiated. An exit interview was conducted, and a copy of the report, along with appeal rights and a Notice of Site Visit, was provided to Director Leanne Sparks. The Notice of Site Visit must remain posted for 30 consecutive days in a prominent location visible to families and caregivers.
