Inspector’s narrative

What the inspector wrote

On 06/19/25, Licensing Program Analysts (LPA) Kelli Waters and Licensing Program Manager Carlos Martinez conducted an unannounced visit to the facility to inspect the infant playground area. LPA met with Deseree Armstead and toured the facility inside and outside. During this visit, LPA Waters and LPM Martinez inspected the playground, but it was determined that it did not fully comply with Title 22 Regulations. LPM Martinez explained that there were several issues that were identified that could potentially pose safety risks to children in care. LPM also reviewed the current waiver on file (Dated 04/06/12) and informed Ms. Armstead that the "outdoor activity" waiver was currently active and limited the census to 6 infants. Ms. Armstead agreed to continue to follow the conditions of the waiver and would be submitting a request for a shared playground waiver in the approved preschool playground. Licensee was asked to submit an updated LIC 308 and an updated LIC 999A facility sketch for the the indoor space and outdoor playground. No deficiencies were observed during this visit. An exit interview was conducted with Licensee Deseree Armstead. The appeal rights were discussed and provided along with a copy of this report to Licensee Deseree Armstead, on this date. A Notice of Site Visit was also provided and must be posted for 30 days.
